Male Pattern Hair Loss / Androgenetic Alopecia

Male Pattern Hair Loss (MPHL), also known as Androgenetic Alopecia, is a hereditary condition affecting scalp hair follicles leading to early follicular degeneration. It mostly occurs after puberty and started affecting frontal hairline, then progress to affect the mid-scalp and crown area sparing temple and back of head area. It is due to Dihydrotestosterone (DHT) which inhibits the normal growing (anagen) phase of scalp hair follicles and resulted progressive shrinking down of hair follicles. The condition will usually stabilize around the age of 50 owing to reduced level of DHT. MPHL is a clinical diagnosis, with neither clinical symptom nor laboratory confirmation.

    Hair transplant has been considered as an effective surgical intervention to treat hair loss problem. It is an autologous tissue transplant by using the donor hair follicles (from back of head) which are less affected by dihydrotestosterone (DHT), then move to bald or thinning area on scalp (recipient site). It has mostly used to treat receded frontal hairline, mid-scalp thinning or even crown baldness.

    Follicular Unit Extraction (FUE), also known as minimal invasive hair transplant, is a modified donor harvesting method. Each individual follicle will be extracted under local anesthesia, and the wound at the extraction will be around 1mm in diameter which will close without suturing.

    Same as FUT, newly implanted hair at the recipient site will shed 1 week after surgery, and the swelling after surgery will subside within few days depending on size of the surgical session. Comparing with FUT, donor site of FUE heals faster and its biggest advantage over FUT is that it will not leave behind a long fine linear incisional line over the back of head. However, the density of donor site will be greatly reduced with each subsequent FUE session, bigger the FUE session, thinner the donor site will be.

     Although hair transplant can fix the appearance resulted from hereditary hair loss, however it will not be able to stabilize the progressive deterioration to the remaining areas. Therefore, it is necessary to start before or at the same time the prescriptive medicine from doctors to stabilize the condition. It is said that the number of hair follicles required for transplant may vary individually subjected to the degree of hair thinning at time of presentation, also the density of donor hair follicles, scalp elasticity, hair texture, previous surgery and different in expectations.     For FUT, an area will be measured and marked out at the back of head and a strip of skin containing hair follicles will be removed, the wound will then be approximated on each edge and closed with Trichophytic Closure Method which will enable the resultant scar unnoticeable. Then nursing staff will dissect the harvested strip of skin with care under power microscope, to each individual follicular units containing either 1 hair, 2 hairs or 3 hairs, then these dissected follicular units will be implanted to the recipient site, namely frontal hairline, mid-scalp and crown area.

    Same as FUE, newly implanted hair at the recipient site will shed 1 week after surgery, and the swelling after surgery will subside within few days depending on size of the surgical session. The wound closed at the donor required suture removal at about 10 days after the surgery.
